The Case Manager will interview and determine eligibility for rental/housing assistance, provide case management services to at-risk or homeless households in Middlesex County, includes service planning, linkage, crisis intervention, de-escalation, managing relationship with a broad range of community providers/landlords, housing retention, completing housing inspections throughout NJ, and utilization of HMIS case management software.
The LifeSet Specialist is responsible for providing services to address the needs of young people ages 17-22 who are transitioning out of resource home/foster care, or are already living independently, and need individualized skill development and support to transition successfully into adulthood.
Works as part of a multi-disciplinary PACT team. Provides community-based treatment and skills training to persons with serious mental illness. Provides valuable input to the team regarding his/her perspective on all aspects of service delivery to consumers.
Provides intensive in-home counseling to CP&P referred families in Middlesex County with 24/7 coverage required. Case management and documentation and collaboration with CP&P and other providers.
