Salary: $45,000 per year Plus Sign on bonus $3,000
Key Responsibilities:
Provide case management services to individuals with mental health illness, who are at imminent risk/currently homeless.
The job includes screening, assessment, community outreach across the county, service planning, linkage, counseling, life skills teaching and utilization of HMIS case management software.
Minimum Requirements:
Bachelor’s degree (B.A) from an accredited college or university, preferably in Social Work or a similar Human Service field is preferred.
At least two (2) years relevant work experience and demonstrated ability to provide effective services to clients required.
Have a familiarity with housing first, progressive engagement, and reality testing.
Experience with conflict resolution and mediation.
Bilingual a plus.
Must have valid driver’s license and clean driving record.
