Case Manager 126 - Flemington NJ
Salary: $50,000 per year

Key Responsibilities:

• Manage all new referrals for the SPIRIT outpatient program and begin their services.
• Work within a team approach to assist clients with overall coordination of basic needs such as Housing need; any financial needs such as social security or general assistance.
• Provide transportation to doctor appointments as needed.
• Conduct group sessions.
• Maintain program statistics.

Requirements:

• Bachelor's Degree (BA/BS) or BSW with some experience with community resources.
• Ability to be flexible with their work schedule.
• Valid driver’s license with an acceptable driving history.
