Case Manager 124 - Full Time
Salary: $41,000 per year

Responsibilities- Working with and supporting family members and caregivers with an adult relative living with mental illness
- Provide case management services, including outreaches, linkages, advocacy, referrals, and general support
- Assist to facilitate multi-family psycho-education and support groups, including researching ongoing psycho-education topics, presentations
- Provides ongoing and communication with open caseload
- Ensure proper documentation in a timely and consistent manner
- Bachelor’s degree in SW, psychology or related degree with a minimum of 2 years as a Case Manager and 2 years working in the mental health field
- Some evening hours required
- Flexible schedule as some duties will take place in the evening hours
- Driver’s license with acceptable driving history a must
