Under the direction of the Clinical Supervisor, the Case Manager in the Adolescent Substance Use Recovery Program is responsible for transporting adolescents and/or parents to and from program; ensuring safety of passengers; following all Agency vehicle requirements; maintaining vehicle; and documenting transportation encounters. The Case Manager will function as part of the treatment team and will communicate with all other program staff, adolescents, parents/legal guardians, etc.to provide multiple modality clinical services to and case management on behalf of the adolescents in treatment. The Case Manager will support connections between adolescents in program and school, work, and/or other extracurricular activities. The position may include individual, family, and/or group psychoeducational services, treatment planning, record keeping, scheduling assistance, case management/recovery support services, and documentation in all required electronic health records. This job requires a basic foundation in clinical knowledge and good communication skills. Much work is done under close clinical supervision.
Salary: $52,000 per year plus Sign On Bonus - $2,500
Key Responsibilities:
Case Manager for OP/IOP adolescent program
Member of treatment team serving adolescents with substance use and co-occurring disorders
Organize and participate in Family/Child Team meetings
Support adolescent and family with relevant school-related issues
Facilitate adolescent/family engagement
May be asked to transport clients to and from program.
Minimum Requirements:
Bachelor’s Degree in mental health, substance use, public health, or a related field
3+ years of relevant experience preferred
